Description (Service)

Provide information, education and support for persons living with dementia and their care partners. The following programs and services are offered:

  • Navigation (connects individuals newly diagnosed and/or who have not accessed other support / community services)
  • Supportive Counselling
  • Care Partner Support Groups
  • Education programs for persons living with dementia, their care givers, professionals and the general public
  • Social, Recreational and Exercise programs
  • Resource Library & Information packages
  • Activity Kits

Programs are delivered throughout Simcoe County.

Members of the public can self refer to the programs simply by contacting the office. A short intake will follow. Community partners and family physicians can refer clients to use using the First Link Referral form First Link Referral

Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm

Summer Hours 2022: Jun 27-Sep 2: Mon-Thu 8:30 am-12 noon, 12:30 pm-4:30 pm * Fri 8:30 am-12 noon, 12:30 pm-2:30 pm

  • Self referrals or referrals from Health Care Professionals
  • referral form is available on website
  • call, email or drop-in to access general information and/or to start the process of accessing the services
  • appointments are required to speak with a Family Support Coordinator
Languages English ; Some literature in French.
