Wrap around health and social supports to underserved community members, with a focus on those who are using substances and experiencing homelessness
Provides outreach service and supports that include;

• Health and medical care
• Peer support
• Mental health and addiction support
• Harm reduction supplies and education
• Housing and shelter system navigation
• Possibly: food, clothing and more

This project is a partnership between United Way of Bruce Grey, CMHA Grey-Bruce, Grey County Community Paramedicine, Rapid Access Medicine Clinic, Grey Bruce Public Health, the Salvation Army and more. Specific agencies attending will vary from week to week.

Target clients:

• Adults who are not currently engaged with traditional health and social supports

and are

• Community members who use or were using drugs

and/or

• Individuals impacted by or experiencing sex trafficking

and/or

• Those experiencing homelessness

Minimum ages is 18 years old.
