Description

Description (Service) Assists developmentally disabled adults living substantially on their own in the community to live as independently and as safely as possible.

Individuals living with family or in residential programs may also be eligible to receive supports for periods of up to one year if they either require some assistance with advocacy or wish to plan to live more independently and require case management supports to achieve their goals.

Services include:
  • case management
  • assistance with budgeting
  • coordinating medical or dental appointments
  • referrals to appropriate resources including but not limited to housing, education, employment and professional counselling
  • assistance with obtaining appropriate assessments to aid in planning
  • support for any involvement with the legal system
  • assistance with finding support for daily activities
  • support to develop the individuals skills in learning how to advocate for themselves

Hours Mon-Fri 8:30 am-4:30
Eligibility
  • 18 or older
  • Deemed eligible and referred by Developmental Services Ontario (DSO) under the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act, 2008.
  • Residing within the HKPR area
  • The APS program is primarily intended to support individuals who live substantially on their own but the APSW guidelines allow other eligible individuals to receive support for a limited time, typically up to a year. This can include individuals living in residential programs or with family members.
