Description (Service) | Team-based family and community primary care.
NFHT Nurse Practitioners (NPs) and Registered Nurses (RNs) provide both urgent and booked appointments for patients requiring primary care medical services. These services are for patients of doctors of NFHT associated clinics.

Application | Call for appointment or to find out how to register for a program or service. To find a family doctor who is accepting new patients: 1. Register with the Health Care Connect service and have a nurse find a doctor or nurse practitioner for you. (https://www.ontario.ca/page/find-family-doctor-or-nurse-practitioner) or 2. Use The College of Physicians and Surgeons of Ontario's Find a Doctor search. Choose "Advanced Search" to find a doctor near you (by city/town or postal code), click on "Additional Search Options" to narrow your search (to family doctors and/or language spoken), contact the doctor to check if they are accepting new patients |
