Description (Service) | Program of the North Simcoe Muskoka Hospice Palliative Care Network offers services and ongoing support to anyone living with a progressive life-limiting illness and/or their friends/family members/caregivers. Support from diagnosis through end-of-life, including bereavement support after a death. Each service has a different area of focus and has been designed to help individuals, friends and family members in specialized ways. Bereavement Support Services:

Hours | Mon-Fri 9 am-4:30 pm
Support Group: Tue 10 am-12 noon |
Areas Served | Chippewas of Rama First Nation ; Orillia ; Oro-Medonte ; Rama ; Ramara ; Severn |
Eligibility | People who need end of life care or bereavement supports. |
Application | Client will complete an intake process, then a Program Coordinator will complete an assessment and make a service match. Call for intake. |
