A collaborative primary health care team providing a full range of primary health care services and programs to the residents of Stratford, Tavistock and Region. Staff includes nurse practitioners, nurses, mental health providers, pharmacists, dietitians, and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient-centred care. Group, family and individual counselling. Offers an After Hours Clinic that rotates between their three clinic sites by appointment only for urgent problems that arise after 5 pm that day and cannot wait until the following day. Programs and Services: Bone Health, COPD Clinic, Diabetes 101 - Understanding the Basics, Diabetes DYAD Clinic, Feeding Your Baby, Flu Shot Clinic, Heart Health Workshops, Maternal Care, Persistent Pain management and Support Group, Raising a Healthy Happy Eater, Smoking Cessation, Sports Medicine, Wellness for Tots, Cognitive Behavioural Therapy for Insomnia (CBT-I), Happiness 101, Intro to Mindfulness, Memory Clinic, Mindful Eating, Mindful Moments (Mindfulness Drop-in Clinic) and Mindfulness-Based Cognitive Therapy.
