Hours | Hours: 8 am-8 pm, 7 days per week |
Areas Served | Hastings County ; Quinte West |
Eligibility | A patient is eligible for enrollment in the Community Paramedic Program if they meet the following criteria:
The patient has a chronic medical condition that is not well-managed, such as:
- Acute Exacerbation of Chronic Obstructive Pulmonary Disease (AE COPD)
- Congestive Heart Failure (CHF)
- Non-Insulin Dependent Diabetes Mellitus (NIDDM)
- and the patient is assessed to be at high risk of requiring emergency medical services due to their medical condition. |
