|Description (Service)||A non-profit health centre focused on family and community health. Health professionals include: a physician * nurse practitioners * registered nurses * a registered dietitian * diabetes educators * a mental health and addiction counsellor * a physiotherapist * a health promotor * and social workers
Provide access to healthcare and treat non-life threatening injuries or illness. Also offer referrals to other local health services and personal development groups.
Also provide health promotion programs including: Fitness for Health * Living a Healthy Life with Chronic Conditions * Living a Healthy Life with Chronic Pain * Mindfulness Meditation * Diabetes Education Programs * Youth Outreach in Highschools * Education Talks * Garden Education Program * Mental Health First Aid * Nordic Pole Walking * Mothercare * Building Balance
The Centre is always looking for feedback on potential workshops to run, so feel free to contact them to make a suggestion. A community-led health team develops these programs to improve the social supports and conditions that affect long-term health.
Services with a Nurse Practitioner are also offered to registered clients in Elmvale on Wednesdays by appointment at the Springwater Health Services Building (Kenzie Knox Building - 35 Queen Street West, Elmvale) from 9 am-3 pm, see separate entry for further details.
|Public Comments||The South Georgian Bay Community Health Centre currently has a waitlist for new clients.
Priority will be given to individuals and families that are currently without a doctor, and who fall into one of the following priority populations:
* Individuals or families experiencing poverty, housing and/or food security challenges
* Youth, between the ages of 12 and 24, with an emphasis on at-risk youth
* Isolated seniors
* Individuals with any disability that presents a barrier to wellness or service access
* Individuals or families experiencing mental health and/or addiction issues.
Note: This is NOT a walk-in clinic.

|Application||For new patients in need of medical care, call reception to get on the waitlist. Partner organizations can refer clients with more urgent needs by calling the main line.
Health Promotion Groups are FREE and open to anyone (you do not need to be a client to register for programs), self-referral, call or drop in to inquire.
