Description
Description (Service) | Provides primary care for individuals who either lack primary care or require more comprehensive primary care than they are currently able to receive from their family physician or nurse practitioner. Available to individuals with a serious mental illness and/or addiction issue for which they are receiving local mental health and/or addiction services. The Nurse Practitioner and Registered Nurse can provide a variety of services, including, but not limited to:

Application | Internal referrals only. Speak to your mental health and/or addiction worker about the program, and ask them to complete a Primary Care Telemedicine referral form with you. Once your referral has been received, you will be contacted by a member of the program. |
