Description
Description (Service) | The Family Support Worker (FSW) provides community case management and resource support to families in Perth County with children under the age of 18 with a developmental disability. The FSW walks alongside families to assess support needs, advocate and liaise, in collaboration with community supports, the school system and medical teams. The FSW can provide supportive counselling, assist with problem solving and service navigation, attend meetings and appointments, assist with funding applications and facilitate case conferences. The FSW works with families to determine a support style that would be helpful. The involvement of the FSW is based on the child's unique needs, and the FSW works with caregivers to develop goals to ensure their child's needs are met and build resiliency. |
